Report: Log of Ohio Inmate Suicide Falsified

A report says one and possibly two prison guards falsified an electronic log documenting checks on an Ohio death row inmate who committed suicide. The report by the Department of Rehabilitation and Correction also says one of the officers did not conduct the rounds as required by orders for that unit. The nine-page report released Monday reviews the Aug. 4 death row suicide of condemned killer Billy Slagle just days before his scheduled execution. The report also says the level of light in death row cells at night is inadequate and says there is an ongoing struggle keeping inmates from blocking the windows with paper and other materials. The two corrections officers named in the review have been placed on paid administrative leave as the prisons agency investigates.
